- • Health conditions (only if relevant for sports participation)
- • Profile photo (for ID cards and official records)
- • Employee ID card copy (for verification)
Event & Logistics Information
- • Sports events you register for
- • T-shirt size and meal preferences
- • Travel plans (for accommodation arrangement)
